1. I acknowledge, if applicable, that by signing this declaration, I represent and warrant to you that I am authorised by the company or organisation named under “Merchant Details ” above(“Applicant”) to do so.
2. I acknowledge that if this application is approved, U Mobile will offer the facility subject to the terms of the Merchant Agreement. I understand the Merchant Agreement, which the Applicant willreceive, will consist of the Letter of Acceptance, the Merchant General Terms and Conditions, the Merchant Guide and the Applicant will become legally bound by these by commencing to processtransactions through the facility.
3. I acknowledge that I have viewed on U Mobile’s website and understood U Mobile’s Privacy Notice (available at https://www.gobiz.com.my) and I consent and authorise U Mobile to collect, record, hold, store, use and disclose the information stated in this Merchant Application Form according to U Mobile’s Privacy Notice.
4. I represent and warrant to you that: -
(a) the contents of this Merchant Application Form and any attachments to it are accurate and true and are given in support of this application;
(b) where I have provided information about another individual, the individual has been made aware of that fact and the contents of U Mobile’s Privacy Notice and I have obtained the consent of theindividual for U Mobile to use and disclose the information in connection with this application.
5. I irrevocably and unconditionally consent and authorise the following credit reporting agencies to process and disclose to U Mobile, the Applicant’s credit worthiness, credit history, credit standing,credit capacity or other credit information for the purpose of assessing this application: -
(a) the Credit Bureau of Bank Negara Malaysia (including the Central Credit Reference Information System (CCRIS), the Dishonoured Cheques Information System (DCHEQS), the PerbadananInsurans Deposit Malaysia (PIDM), the Financial Institutions Statistical System (FISS) or any other database or system established by Bank Negara Malaysia, Credit Bureau (M) Sdn Bhd, Credit Tip-OffSystem Sdn Bhd, RAM Credit Information Sdn Bhd; and
(b) any other credit reporting agencies duly registered under Credit Reporting Agencies Act 2010.
Where the Applicant is a Partnership, signing must be in accordance with the Partnership Agreement.
Where the Applicant is a Company, signing must be in accordance with the Company Constitution.
Where the Applicant is a Sole Proprietor, the proprietor must sign.
